You need to have Dodge Counter (I'm not sure about Dodge Kill and Dodge Kill mastery, some people claim you have to) skill unlocked to be able to get this. Once near a Swordsman, start pressing "C" to initialize Dodge Counter once he attacks you, the time will slow down and you will be prompted to press a button at the right time. Once you perform that successfully the ach will pop, Deathless doesn't need to die even, as he didn't since I was playing on the hardest difficulty level, but stil got the achievement after stabbing.

    Unlock conditions:

    Dodge a melee attack from a Deathless Swordsman and confirm the QTE prompt well timed (shortly before the white moving ring reaches the innermost circle) and Lara will perform an animation that will have her pierce the foe's head with an arrow and kill them instantly.
