    You get 5 hearts every day, they get restored at 3pm Japan time.
    If you die without any hearts, you lose all your equipped equipment and get this achievement.

    You dont lose equipment from your inventory or storage warehouse, dont worry, you do lose hardcore mode status if you die though.
