Find 2-3 specific items and fully upgrade them, after collecting a chest you can get the chance to evolve the item.
If you do not have the specific item to evolve, then look at the description on the achievement to see what you need to do.

Note: These are all weapons so you need to have three weapon slots available when evolving this without using Eleanor.

These are caterpillar mobs, you see 1-2 of them at the start of the 'Lake Foscari' map.

To get more - start the 'Lake Foscari' map, go to where the Seal of the Lake crystal is/was. Then go south alongside the right side a bit, there's a tree line on the right side that has spots where you can enter. Once you do, there's Sammies all over there.

If you've reached the right metaglo, you've gone way too far.
