    This achievement could be a bit buggy. If you've already unlocked all the other achievements but this one doesn't trigger replay the last level (stage 10) and defeat the final boss with all 3 hearts (like for Too easy... achievement). If you get hit during the boss fight just jump into lava and that will restart the boss stage.
