After trying a few times I finally got it and I wanted to share some additional observations.
I did this on a New Game+, after finishing the main story, you restart the game with all your things and you no longer have to shoot down the key.
I first tried with the bowgun since someone mentioned you could pull this off with it but it didnt work for me.
I also thought that hitting a monster when you were grabbed would invalidate this achievement but it didnt.
I am pretty sure that kicking a zombie down with Claire in one of my runs invalidated the run.
Also breaking a crate with Moira's crowbar.
I learned that the best way to avoid melee attacks is to hold down the knife attack button instead of clicking it every time you want to slash.
Holding down the attack button won't trigger any finishers, sneak attacks or knockdown attacks, so you can hold down the button and walk towards your target.
You can crouch beneath the spinning blades in the flamethrower room so the horde dies trying to get you, you dont need to turn the flamethrower on with this and knowing this saved me a lot of grief in these knife runs.
Healing is ok.
I read retrying the stage wont invalidate the achievement but I got it with no deaths, so if it isnt unlocking try to avoid retrying.
I also avoided using Moira's flashlight and Natalia's pointing.
I didnt kill the cabin revenant in Barry's episode, I just ran outside and slowly backed away to the door to open it, he grabbed me a couple of times but I had a lot of herbs and on casual they dont do much damage. Natalie handled powering up the crane and although AI Barry got grabbed a couple of times, he wasnt even close to dying.

You can also do this on Barry's episode 2 as you enter the warehouse through the double blue doors, right after the checkpoint just walk down the stairs, open the door and the revenant will have its back towards you, he wont even notice you are there so you just need to shoot his arm off. Pause, Retry, repeat.
This was a faster way because I didnt have to wait for the gate cutscene also when you try with the above method, you spawn back in the cabin where you get the alcohol and the bottle and not the gate, so you need to wait for the cutscene of you going down the steps, then you have to run to the gate, then the gate cutscene and then you fight an alert revenant, with the revenant on episode two, its way, way faster.

0- There's a Medal for That
There are several tricky medals for this episode but these two are the more challenging to get:
For Claire:
Cleared the siege in the bar without any of the windows being broken.
During Claire's side, you will be attacked by a horde of afflicted and they will try and break the window steel protectors to get inside, to get this medal you must not let a single barrier fall, they can get damaged but they should not come off. Its easier to do this with plenty of ammo and on Casual difficulty, you need both firepower and speed, avoid reloading if you can or get good reloading and capacity parts on your weapons. If you see a window come off, just pause and retry.
The rest of Claire's medals are pretty straightforward.
For Barry:
Defeated 2 enemies with a follow-up attack.
Oh boy, this one is a pain in the butt.
The only way to get a follow-up attack prompt while an enemy is on the ground in this side of the episode is to allow a slinger (big enemy that slings his arm parasite thing) to knock down a Revenant. At that time, you'll get the follow-up melee command when standing near the fallen Revenant BUT you need to do it in such a way that your dagger hits the revenant's weak spot as it goes into the ground which is so.. so very tricky.
Also, if the slinger's attack lands near the Revenant's exposed weak point, it might kill it instead of knocking it down.
So, to kill a revenant with a follow up attack you need to:
Expose its weakpoint.
Get close to it and dodge its attacks while the slinger aims at you and throws its arm.
Hope the attack doesnt kill the revenant and knocks him down.
Approach the revenant from such an angle so you can see the button prompt and when the dagger goes down, it hits the exposed weak spot, killing it.
Now, I tried this for an hour or so until I finally noticed that its easier to do this to revenants with their heads as weakpoints, since the slinger's parasite bomb thing most likely will hit the floor near the revenant or its back. If the revenant has its weakpoint in its legs, the explosion will kill it.
Natalia's flying brick attack will knock down the revenants but it will not give you the prompt for a follow up attack.
Playing on Casual and having herbs and disinfectant helps a lot.
The slingers are in adjacent areas, one in the warehouse and the other outside, just before entering the medical facility.
Each of these areas should have a revenant with its head as a weakpoint.

0- Unfriendly Skies
I personally used this method to get the achievement so I can confirm it works on the current steam version.
You can check if its working by pausing the game, going into gameplay stats->records and scrolling down to the name of the achievement.
The first method is also a good place to try and go for Line 'Em Up! since two infected come through the door right before the window section and you can try and align them correctly as they walk towards you. Make sure to bring your magnum.
The second video might also work and its in chapter 2 after separating from Neil, you come to a small bar where an infected close to the roof by a beam, you can snipe him with the magnum and it should also count.

This method takes less then a minute for each kill using the retry option.
While playing the first part of the episode as Claire and Moira you will be wandering the corridors after the mysterious woman has spoken to you about the bracelets. You will eventually come to an area where 2 enemies come from 1 door DO NOT go around the next corner until you have killed these 2
Make sure you are Claire and have your Shotgun fully loaded after killing the 2 enemies and as you turn the corner an Afflicted will leap through a window in front of you as he hits the floor pump both shells into him and despite him being on the ground the tracker will count this as a mid jump kill. It's important you don't allow him to move or it won't count so you need to shoot fairly quick to get him down. Have your tracker up and you will see it go up despite him clearly not being in mid air, then simply retry and you will be back slightly before the flying Afflicted and you can just rinse and repeat until the Achievement pops.
You can of course use an explosive bottle to kill the Afflicted, whatever works best for you
