This one requires you to correctly diagnose the patient with everything (all conditions and all treatments). This can only be done on a 75+ year old patient.

The game generates patients randomly, and the conditions and treatments are determined for each patient before you begin scanning them. The fastest way to get this achievement is to replay level 24, and just pass all patients that aren't 75+ AND have lifestyle advice (which doesn't require any scanning to determine). It took about 15 gameplay hours for me - it might take more or less for you since it's completely random.
