Achievement is broken on the latest build - the Halloween Costume and empty eyes are not available when creating a new save during the Halloween event and not found on the store for purchase. The outfit is necessary for the achievement.

You'll need to download and use an older version of the game. I found the April 2017 manifest to work:

Achievement requirements:
Create a new profile during the Halloween event
Select Halloween bloody outfit - shirt, pants, shoes. Select empty/glassy eyes.
Make your character a Musician, it might not trigger with other professions.
Progress through the game until asked to move in with someone.
Once you move, purchase the pumpkin decoration on the computer and purchase 1 Halloween food item from the fridge.

Note: The different versions don't fully recognize each other's saves so a backup is recommended. I recommend not launching a new version save on the 2017 version. You can load the old version save on the latest build and save it to make it fully recognized. The Halloween outfit from the old version save was on the character when loaded on the latest version.
