Start Single Player > Campaign mode with the following settings:

Infinite Health
Infinite ammo
Cooldown - minimum
Wave size - minimum
Number of waves - minimum
Difficulty - minimum

Complete all 5 missions for 100%.

Mission 1

• Go inside the bar.
• Play the piano.
• Finish all waves.

Mission 2

• Go outside.
• Enter the Undertaker building
• Finish all waves.

Mission 3

• Go outside
• Enter Cemetary to your right.
• Finish all waves.

Mission 4

• Go outside.
• Enter Church to your right
• Read the book.
• Finish all waves.

Mission 5

• Go outside.
• Enter Sherrif's building
• Read the note hanging on wall in the second jail cell.
• Finish all waves.
