You need to die 5000 times for this achievement. If you're really bad at the game you might get this naturally. But most people seem to need to manually grind this out. Luckily, it doesn't take long. I was at ~1300 deaths after beating the game, went to level 11, and held down my left button ('a' key) for a bit. It took me only ~30min to get 3700 deaths. Doing a quick calculation yields that doing all 5000 deaths would take ~40min in total. So regardless of how many times you died, it won't take you more than 40 minutes to complete this achievement. I just put something heavy onto my 'a' key and left my PC for a bit.
